    The effect of a structured internship on the clinical performance of novice nurses?

    Objective: The study examined the clinical performance differences of novices before and after a critical care internship and compare their performance attributes to a reference group comprised of experienced critical care nurses. The extant research regarding clinical internship programs has focused on retention in practice, workforce issues and competence. While studies have shown a pattern of improved retention in practice settings, they have failed to objectively measure actual clinical performance. Methods: The study design was based on pre-post comparisons of novice nurses and a reference group comprised of experienced nurses who served as a benchmark. The novice group was required to respond to patient needs in high-fidelity clinical scenarios before and after an internship. The experienced nurses attended a single testing session. Results: Findings revealed statistically significant improvement in the performance of the novice group; however, they failed to perform at the level of the experienced nurses. Conclusions: The study is the first to detail the degree to which a structured internship can facilitate changes in selected aspects of clinical performance. The study establishes the effect associated with engagement in a structured internship. While novices improve significantly, they fail to achieve the level of clinical performance seen in experienced nurses.     Design and implementation of high-radix arithmetic systems based on the SDNR/RNS data representation

    This project involved the design and implementation of high-radix arithmetic systems based on the hybrid SDNRIRNS data representation. Some real-time applications require a real-time arithmetic system. An SDNR/RNS arithmetic system provides parallel, real-time processing. The advantages and disadvantages of high-radix SDNR/RNS arithmetic, and the feasibility of implementing SDNR/RNS arithmetic systems in CMOS VLSI technology, were investigated in this project. A common methodological model, which included the stages of analysis, design, implementation, testing, and simulation, was followed. The combination of the SDNR and RNS transforms potential complex logic networks into simpler logic blocks. It was found that when constructing a SDNRIRNS adder, factors such as the radix, digit set, and moduli must be taken into account. There are many avenues still to explore. For example, implementing other arithmetic systems in the same CMOS VLSI technology used in this project and comparing them to equivalent SDNR/RNS systems would provide a set of benchmarks. This information is not always available, necessitating the provision of methods to estimate the time since catch or harvest. The objectives of this study were therefore to develop and/or validate sensory and ATP derivative-based methods for rapidly assessing the freshness of fish. A quality index method (QIM; raw fish) and a quantitative descriptive analysis (QDA; cooked fish) were developed and validated (against bacterial count [total viable count (TVC)] and time) for salmon (Salmo salar) and cod (Gadus morhua). The production of inosine monophosphate (IMP), inosine and hypoxanthine (Hx) and associated ratios (IMP/Hx, K1-value or H-value) were also investigated for use as freshness markers. There was a linear relationship between QIM and TVC (R2 = 0.93 for salmon and R2 = 0.89 for cod), QIM and time (R2 = 0.96 for salmon and R2 = 0.98 for cod), QDA and TVC (R2 = 0.93 for salmon and R2 = 0.77 for cod) and QDA and time (R2 = 0.94 for salmon and R2 = 0.87 for cod), suggesting that the QIM and QDA schemes developed could be used to monitor/assess freshness. The H-value also increased linearly with TVC (R2 = 0.88 for salmon and R2 = 0.89 for cod) and time (R2 = 0.93 for salmon and R2 = 0.84 for cod). It was therefore concluded that both the QIM/QDA approach and monitoring ATP degradation, specifically expressed as the H-value, could be used as rapid methods to assess the freshness of salmon and cod arriving at the processing plan

    Probabilistic model for contamination of egg dishes with Salmonella spp. made from shell eggs produced on the island of Ireland

    A quantitative model was constructed to estimate the probability that a serving of food containing eggs produced on the island of Ireland is contaminated with Salmonella spp. The model is based on the prevalence of contaminated eggs at the time of lay and a set of parameters which describe the pooling of eggs in the home and in catering situations. Both external and internal contamination of the eggs by Salmonella spp. was considered. The model estimates that there is a 90% chance that the probability of a serving of food being contaminated is between 0.0043% and 0.038%. Sensitivity analysis demonstrated that egg prevalence drives this low probability and that, at the current level of egg prevalence at the time of lay, pooling of eggs has a minor effect. These results indicate the importance of maintaining the low prevalence of contaminated eggs at the time of lay to minimise the risk of human cases of salmonellosis from consumption of eggs

    Qualitative exposure assessment for Salmonella spp. in shell eggs produced on the island of Ireland

    A qualitative exposure assessment for Salmonella in eggs produced on the island of Ireland was developed. The assessment was divided into three main modules (production and packing, distribution and storage, and preparation and consumption), and each of these stages into defined steps in the exposure pathway. In the production and packing stage the initial prevalences of Salmonella in the contents and on the shell of eggs were estimated to be negligible and low respectively. Numbers of Salmonella both in and on eggs were estimated to be low. At each subsequent step in the pathway, qualitative assessments were made of the impact of events on the probability and level of Salmonella contamination on the shells and in the contents of eggs. At the end of each module assessments were combined to give an overall probability and level of Salmonella contamination. In the first two modules the assessment focused on the effect of the duration and temperature of storage on yolk membrane integrity and the likelihood of shell penetration. During the final stage the influence of factors such as safe handling procedures, pooling practices, consumption patterns and the effectiveness of cooking, on the prevalence and level of Salmonella contamination in a food item at time of consumption was assessed. The outcome of this assessment was an estimate of a low probability and level of Salmonella contamination of egg containing foods, prepared with eggs produced on the island of Ireland

    Girls and Young Women in the Girls and Young Women in the Youth Justice System – Vulnerable or Risky?

    This short briefing paper examines the complexities around identification and effective intervention for girls and young women viewed as vulnerable and who may be at risk of future offending. The CJSW Development Centre’s National Development (Champions) Group for vulnerable girls and young women undertook a profiling exercise in an attempt to obtain a better understanding of the needs and vulnerabilities of girls and young women involved in, or on the periphery of, the youth justice system. While limitations of the data collection tool and methodology preclude wide generalisations, the complexities in attempting to identify individuals who may be at future risk of offending based on assessment of present and past needs and vulnerabilities is apparent. Despite the identification of often substantial vulnerabilities and needs in a sample of 12 to 16 year olds, it appears the majority of girls and young women were not involved in serious or persistent offending behaviour. The findings have implications for policy and practice development, which are now focussed on the development of effective and early interventions for all young people considered to be at risk of future offending

    The effect of antimicrobials on verocytotoxin bacteriophage transduction under bovine rumen fluid and broth conditions

    peer-reviewedThe verocytotoxin genes in verocytotoxigenic Escherichia coli (VTEC) are carried by bacteriophages, incorporated into the bacterial genome (prophage). Antibiotics may promote phage replication and release to infect other cells (transduction), thus leading to the emergence of new VTEC strains. This study investigated transduction of a verocytotoxin2-encoding bacteriophage (3538(vtx2::cat)) under laboratory conditions, including the effect of antibiotic treatments. Luria-Bertani Miller broth and rumen fluid (raw and sterilised by irradiation) were inoculated with the donor (C600φ3538(Δvtx2::cat)) and recipient (E. coli C600::kanamycinR) strains (4 log10 cfu/mL) and incubated at 38°C. Antibiotic treatments (minimal inhibitory and sub-inhibitory concentrations of ampicillin, cefquinome, oxytetracycline and sodium sulfamethazine) were applied after 3 h. Samples were tested for donor, recipient, cell-free phage and transductants at times t = 0, 3, 4, 6, 27 (24 h post-antibiotic treatment) and 51 h. Free phage was detected in the untreated broth and rumen samples, as were the transductants confirmed by polymerase chain reaction. The antibiotic treatments did not significantly (P > 0.01) increase the concentrations of free phage or transductants detected. It was therefore concluded that, under laboratory conditions, the antibiotics tested did not induce bacteriophage lysis, release and infection of new bacterial cells beyond that constitutively found in the phage population

    Quantifying the health and economic benefits of active commuting in Scotland

    Background: Despite the substantial evidence base for the health and economic benefits of walking and cycling, there remains a lack of published findings on the levels and benefits of active commuting at a national level. This study aimed to quantify the proportion of active commuters who met a daily equivalent of weekly physical activity recommendations through their commuting journeys, and the economic value of health benefits associated with active commuting in Scotland. Methods: A repeat cross-sectional analysis of the 2001 and 2011 waves of the Scottish Census was conducted. We analysed data from approximately 250,000 respondents aged 16–74 at each time-point who selected walking or cycling for their usual journey to work. A count was taken of walkers and cyclists whose daily commuting time was at least 30 min. The Health Economic Assessment Tool was used to estimate the number of deaths averted by active commuting, and the associated economic value of walking and cycling annually and over a 10-year period. Results: Active modes of commuting accounted for a modal share of 13.5% (n = 244,009) in 2001, and 14.5% (n = 286,145) in 2011. In 2001, 46.5% of all active commuters met a daily target of 30 min of moderate intensity activity rising to 50.2% in 2011. In Scotland, the annual health economic benefit of commuting to work by walking was estimated to be approximately EUR 700.2 million, and EUR 79.8 million for cycling to work. Conclusion: This study provides clear evidence of the substantial health and economic benefits that active commuting makes at a population level. These findings support the case for further investment to increase levels of walking and cycling

This was late August 1971. After a few pleasantries, it became clear that this was not just the courtesy call of a new dean reporting in to the top leader of the state university. Chancellor Boyer went right to the point: a new Labor College was going to open on the premises of Local 3 IBEW’s training facility on Lexington Avenue in Manhattan, and the ILR School had to be there as a partner.
